PRN Hospice Chaplain

Job Description:

Provide spiritual support to hospice patients on an as needed basis. Must have a Bachelor’s Degree in Counseling or related topic OR have one unit of Clinical Pastoral Education (CPE) and Be ordained, commissioned or credentialed according to the practice of an organized religious group2.

PRN Speech Therapist

Job Description:

Provide Speech Therapy to patients on as needed basis in their home. Assessment, care planning, treatment, and documentation along with communication for full-team treatment plan.

PRN Social Worker (LMSW or LCSW)

Job Description:

PRN Social Worker will assist full-time SW for vacations and high census as available. Provide emotional support for hospice patients and families and resource support for home health families as needed. Must have Bachelor’s of Science degree (preferably a Master’s of Science degree) from a social work curriculum accredited by the Council of Social Work Education or better.
